What is a Family Health Team?
A Family Health Team is an approach to primary health care that brings together different health care providers to coordinate your care. Our Family Health Teams consist of doctors, nurses, nurse practitioners, social workers, dietitians, and other health care professionals who work collaboratively, each using their experience and skills to provide care.
The Family Health Teams provide a wide range of health programs and service, including for people who don’t have a doctor. You can get a wide range of health services from a Family Health Team, including advice and guidance on living healthier to avoid illness, or manage a chronic disease.
