about the role:

Sherbourne Health and The Neighbourhood Organization are the co-lead organizations in the Health Access Executive table with 15 other health and social service providers. The Health Access Expansion Project will focus on expanding the existing Health Access St. James Town model to surrounding neighbourhoods.

The Case Worker works closely with the Intake and client engagement lead and supports the project goals by reaching out to the targeted clientele by enhancing and improving access to health care and other community support services. The role also focuses on coordinating and aligning with the client’s healthcare needs, including equitable pandemic response and sustainable healthcare response to residents, specifically those facing multiple barriers to care and social support, focusing on black and indigenous populations.

The Case Worker understands the impact of the broader social determinants of health on people’s ability to attend to their health and well-being. The case worker connects with hard-to-reach and vulnerable individuals in the downtown east neighbourhoods to provide ongoing support to enhance clients’ independence through skill enhancement and development by building a support network and linking clients to available community services.

The Case Worker will closely work with hospitals in Downtown East and provide case management support to the clients who would require community service support after discharge from the hospitals. They work as a member of an interdisciplinary team and provide crisis intervention, case management support and supportive counselling to clients. The case worker works from a harm reduction and anti-racism/anti-oppression framework.

 

day to day responsibilities:

  • Completes social assessments and histories as needed.
  • Connect with hospitals and service providers to identify the clients who need support with independent living after discharge from the hospitals and provide short-term case management support.
  • Assists service users in identifying and prioritizing goals and areas of development that support wellness, empowerment, and autonomy.
  • Develop case management strategies to respond to the needs of clients who tend to visit Emergency hospital care and establish better support systems in the community they are living in.
  • Collaborate with Health Access partners and other external service partners to advocate for client needs, make referrals and provide follow-up to ensure that a connection has taken place.
  • Work closely with primary health care teams to connect unattached clients to health care.
  • Provide coordination of care, promote and support clients’ participation and inclusion in the community.
  • Provide short-term supportive and crisis counselling to a broad range of individuals with mental health and substance use issues.
  • Build strong partnerships with various community organizations to provide optimal harm reduction services and safer drug use education.
  • Record case information, complete all necessary forms accurately and produce statistical and case reports.
  • Accompany clients to appointments as needed.
  • Maintain up-to-date knowledge of community resources, as well as health and social issues in the community.
  • Develop and maintain cooperative relationships with partner agencies and service providers providing programs and services for the populations served.
  • Demonstrate a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with related policies, health and safety legislation and best practices.
  • Participates in the development, implementation, and evaluation of programs.
  • Participates actively in all relevant meetings, committees, special events, etc.
  • Conducts outreach to the targeted population.
  • This position requires the ability to work flexible hours, including evenings and weekends
  • This position requires working off-site in designated neighbourhoods.
  • Other duties as required.

 

To thrive in this role, you’ll need

education/experience/skills/abilities:

  • BSW or equivalent education and experience in health/social sciences or equivalent formal training and 3 – 5 years’ experience working in a community health setting or equivalent blend.
  • Minimum of 1 to 3 years of experience working with newcomer and diverse populations, including 2SLGBTQ communities and persons experiencing homelessness and who use substances.
  • Experience working with populations facing multiple barriers to equitable care.
  • Analytical skills to identify and resolve problems, assess client needs, formulate recommendations, develop appropriate service contracts, and determine actions to address the needs of the client decision-making skills to assess the effort required.
  • Crisis intervention skills.
  • Excellent communication skills in English, both written and verbal.
  • Fluency in a second language spoken in the community is an asset.
  • Excellent organizational and coordination skills to respond to fluctuating workloads and immediate crises.
  • Excellent interpersonal skills.
  • Ability to work within an anti-oppression framework; understand and appreciates the diversity of our client and staff population and community.
  • Ability to work within the harm reduction framework and support clients, experience regarding harm reduction practices and knowledge of harm reduction strategies.
  • Proficiency in using computers, including word and data processing, designing flyers and presentations, projectors, cellphones, phones, fax machines, printers, and photocopiers.
  • Cultural sensitivity and experience working with diverse communities.
  • Experience working with individuals with mental health and addiction issues.

  • The position will be located at The Corner and other sites in the Downtown East.
