 about the role:

The Family Resource Worker collaborates with 2SLGBTQ parents and families in a process that focuses on providing and accessing quality health care and support services. The FRW will act as a resource to 2SLGBTQ communities, service providers and others regarding the needs of 2SLGBTQ parents, prospective parents, their families and children.

The FRW will have a focused role in planning, delivering and evaluating programs and will facilitate sessions of the 2SLGBTQ family planning courses.

scope and mandate:

The FRW will maintain current knowledge of the issues and barriers faced by 2SLGBTQ parents and prospective parents. They will work with the understanding of the interface between the social determinants of health and the community’s health and wellbeing. They will facilitate and monitor the client’s access to appropriate health and social services towards goals set by the client. They will also plan and facilitate sessions of the 2SLGBTQ family planning courses, as needed.

day to day responsibilities:

Information and Referral

  • Supports 2SLGBTQ prospective parents to make informed family planning decisions – provide current/high quality information on options; introduce to the community of peers
  • Provides brief supportive counselling to clients in distress, when necessary.
  • Networks with internal and external service providers to build collaborative, efficient and sustainable relationships to support client goals.
  • Maintains client data, prepares reports and other documentation, as required.

 Program Planning and Group Facilitation

  • Facilitates in person and online groups and workshops to provide information on family planning options for 2SLGBTQfolks and introduce to community of peers.
  • Conducts outreach to targeted participants for group.
  • Conducts evaluation from program participants at the end of program session.
  • Maintains participant records and ensure they are reported within prescribed timeframes.

 Administration

  • Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with SHC related policies, health and safety legislation and best practices.
  • Liaise with and provide data and information to the Fund Development and Communications team as it relates to LGBTQ2S CP, and in particular, their clients and program.
  • Operates within established policies and procedures.
  • Participates in the ongoing evaluation of their programs and service delivery
  • Maintains current knowledge of relevant government acts and legislation.
  • Participates in Sherbourne meetings, committees, and special events, with the approval of the Manager.
  • Performs administrative tasks as required

To thrive in this role, you’ll need

 Education/experience/skills/abilities:

  • Post-secondary education in health/social services or equivalent formal training or at least 3 years work experience with 2SLGBTQ Parents and 2SLGBTQ Prospective Parents.
  • Current knowledge of health, legal and social policies that are relevant to 2SLGBTQ parents and families and ability to make informed interventions and referrals.
  • Demonstrated knowledge of 2SLGBTQparenting and family issues and broader social issues for diverse 2SLGBTQ communities.
  • Experience and demonstrated understanding of working within an anti-oppression framework.
  • Experience with group facilitation including program planning, facilitation and evaluation.
  • Excellent communication skills; ability to adapt communication style to work
  • Maintains participant records and ensure they are reported within prescribed timeframes. General and Administration
  • Understands and works within the Occupational Health and Safety Act (OHSA), Personal Health Information Protection Act (PHIPA), Accessibility for Ontarians with Disabilities Act (AODA), the Ontario Human Rights Code and other relevant legislation

working conditions:

The FRW works in a healthcare and community-based setting. Given the nature of the role, they may face interruption and disruption to any established daily routines. Evening and/or weekend hours will be required. It is crucial that all client and organizational information is kept confidential at all times.
