Youth in Transition Program Data Reporting Software Exploration: A Shared Sector Initiative
About the Project Partners
Youth in Transition (YIT) programs funded by the Ministry of Children, Community and Social Services (MCCSS) support young people navigating the transition to adulthood. These programs are delivered by a network of community-based agencies across Ontario, each operating within distinct organizational, geographic, and population-specific contexts.
This project is a shared endeavour among participating YIT-funded agencies and is being coordinated by Sherbourne Health. Sherbourne Health is seeking a qualified consultant on behalf of the YIT sector to support this work, grounded in a collective commitment to strengthening accountability, improving outcome measurement, and enhancing data-informed practice across YIT programs.

What we want to do:
YIT-funded agencies recognize the growing need for a more consistent, efficient, and meaningful approach to data collection and reporting. Current practices vary widely across organizations, creating challenges related to consistency, staff workload, comparability of outcomes, and sector-wide learning.
Sherbourne Health, in collaboration with YIT partner agencies, is seeking a consultant to conduct a Data Reporting Software Exploration to support the development of a future-state vision for YIT data reporting software that is flexible, scalable, and responsive to diverse program realities.
Purpose of the Engagement
The consultant will work with Sherbourne Health and participating YIT agencies to:
- Assess current data collection practices and reporting requirements
- Identify opportunities for alignment and standardization
- Explore software and system options that support accountability and learning
- Propose a future-facing, scalable approach to YIT data reporting that minimizes double entries and track youth outcome over time
Scope of Work
1. Review of Current State
Assessment of existing software use, data tools, workflows, reporting requirements, capacity constraints, and privacy considerations.
2. Opportunities for Sector Alignment
Identification of shared data elements, efficiencies, and flexible models for coordinated reporting.
3. Environmental / Sector Scan
Review of comparable systems, best practices, innovations, and sustainability considerations.
4. Stakeholder Engagement
Engagement with YIT workers, program managers, organizational leaders, and other relevant stakeholders.
5. Recommendations and Future-State Vision
Development of a feasibility and recommendation report outlining system options, cost projections, funding scenarios, and next steps.

Skills and Experience
- Experience in community services and/or youth-serving sectors
- Advanced knowledge of data reporting databases, case management systems, and reporting software, including their application in multi-agency environments
- Strong background in outcome-based evaluation
- Experience with multi-agency or sector-wide initiatives
- Knowledge of data governance, privacy, and ethics
- Strong facilitation and analytical skills
Timeline
The project is expected to be completed within three months of contract award.
Budget
Funding of up to $26,250 (CAD) is available for this project. This amount represents the maximum total budget and includes:
- Consultant fees (research, engagement, survey development and dissemination, and reporting): up to $23,500
- Survey dissemination costs: up to $1,000
- Stakeholder engagement meetings and report dissemination: up to $1,750
Proposals must be submitted within this total budget and include a detailed cost breakdown aligned with the categories above. Costs exceeding the maximum available funding will not be considered.
