Health Equity Data Collection
Sherbourne Health collects health data information from our clients. Learn more about what that means.
Sherbourne Health is a dynamic provider of integrated health services, community programs and capacity-building initiatives that enable people and diverse communities to achieve wellness. When we collect health equity or sociodemographic data, we better understand the needs of our clients and their health concerns.
What is Sociodemographic Data?
Sociodemographic data is information collected about characteristics of a population, like gender, age, and income for example. Using sociodemographic data, we can reduce health inequities based on language, disability, and other factors.
What is Health Equity?
Health Quality Ontario states that “health equity allows people to reach their full health potential and receive high quality care that is fair and appropriate to them and their needs, no matter where they live, what they have or who they are.”
During your next visit to Sherbourne Health, you may be asked to fill out a questionnaire asking questions about your age, race, ethnicity, gender, sexual orientation, income and more. Participation is completely voluntary, and choosing not to fill in the questionnaire, or not to answer specific questions will not affect your care or access to services.
Through our clients’ answers to these questions, we hope that the gathering of sociodemographic data helps us to advance health equity by giving us the tools to understand their needs.
If you use any of these programs at Sherbourne Health, you may print and fill out a completed form (PDF) to your next visit:
- primary care
- Acute Respite Care
- Diabetes Education
- Hep C
- Mental Health Counselling
- Safer Opioid Supply (SOS)
The form can be given to a medical secretary or your care provider.
