Summary Description
The Manager, Operations and Population Health Management plays a critical role in attaining population management goals for the attributed populations. Reporting to the Executive Director, this role will focus on enacting the day-to-day operational directives for the DET OHT. This includes but is not limited to: liaising with, and onboarding, community partners to the DET OHT; coordinating and preparing reports required by the Ministry of Health and/or Ontario Health; supporting the implementation of the strategic plan across the OHT; and supporting the core group and other working groups as needed.

Population Health
- Monitor population health indicators

- Partner with the Executive Director to implement the DET OHT’s strategic priorities and operational plan.
- Coordinate operational reporting and ensure compliance with Ministry of Health, Ontario Health, and other reporting requirements.
- Lead operational planning, forecasting, and performance monitoring, establishing annual priorities, benchmarks, and regular reporting to measure progress against objectives.
- Review operational goals and plans to ensure alignment with strategic priorities and available financial resources.
- Develop, implement, and maintain operational policies, procedures, and governance processes.
- Build and sustain collaborative relationships with OHT members, community partners, and system stakeholders to advance integrated care initiatives and improve health outcomes.
- Coordinate the onboarding and engagement of new partner organizations and support the work of the OHT Core Table and its committees.
- Identify operational risks and emerging issues, recommending solutions and escalating concerns as appropriate.
- Prepare operational reports, briefing materials, and updates for review by senior leadership.
- Support the development and implementation of the annual operational plan, including work plans, objectives, and performance measures.
- Oversee staff supervision responsibilities, including recruitment, onboarding, performance management, coaching, and employee relations, where applicable.
- Chair or facilitate team and committee meetings as required.
- Participate as an active member of the DET OHT Management Team and other internal and external committees.
- Perform other duties consistent with the scope and level of the position.

Ability to manage and plan annual budget expenditures. Ability to identify and surface potential problems and take proactive action to address issues before they become bigger. Ability to champion new initiatives, lead the change process, collaborate and form strong relationships across the OHT, and create an environment to achieve a desired change.
- Organizational Planning: ability to lead streamlined processes to make them more efficient, and work with others to develop and adopt new procedures and processes. Participate and/or lead in management team meetings, special committees and working groups, as required.
- Community Engagement: ability to foster and build strong relationships with community and funding partners, as required. Explore integrated and strategic partnerships that enhance services to the OHTs attributed population. Initiate evaluations, needs assessments, and surveys that assess emerging population needs/trends.
- Commitment to cross-agency integrated program planning: ability to work co-operatively and collaboratively within the management team, working groups, or in regional/provincial work groups/ committees. This includes contributing fully and actively to team activities, projects, and program initiatives to achieve group and OHT goals. Resolve conflicts by facilitating the expression of diverse points of view to enhance teamwork and attain a beneficial resolution. Take steps to help others resolve the conflict and maintain trusting relationships. Promote co-operation between program teams.

Qualifications:
Skill & Abilities
- Undergraduate degree in a health-related discipline, health policy, business or finance
- Master’s degree in health administration preferred
- Previous experience working in a fully or partially government-funded organization required
- Minimum 5 years of experience in an operational, managerial role that included the management of finances and budget development
- Previous experience applying for and securing funding for strategic initiatives
- Previous experience working with multiple stakeholders across the health and social services sectors
- Previous experience working with both the Ministry of Health and Ontario Health required
- Experience working in a leadership capacity across multiple organizations required.
Knowledge & Experience
- Demonstrated experience leading a large complex collaborative initiative involving multiple, diverse stakeholders.
- Demonstrated excellence in interpersonal, written, and verbal communication skills including, ability to demonstrate tact and diplomacy, manage confidential information, and build rapport and positive working relationships.
- Demonstrated understanding of the health sector, including OHTs, primary care, health equity, acute care, home care, long-term care, and community support services
- Demonstrated advanced computer skills (Microsoft Office – Teams, PowerPoint, Word, Excel, Planner, Virtual Meetings – Zoom, Microsoft Teams)
- Proven ability to work independently and as part of a team, demonstrating initiative and collaborative skills.
- Exceptional organizational skills with the ability to manage multiple priorities.
- Commitment to health equity, anti-racism, anti-oppression, and client and family engagement skills and abilities:
- Proven ability to meet tight deadlines and achieve results when outcomes are not clear.
- Demonstrated experience and success leading partnership projects and initiatives.

Conditions
- This position requires working in a fast-paced environment meeting the needs of multiple stakeholder. Time management, multi-tasking, initiative, organization and prioritizing are essential. Work is mainly performed in an office environment. Occasional work outside of normal working hours may be required, in order to meet with clients, vendors and partners.
