about the role:
The Program Manager of Primary Care Transformation is responsible for the operational management and implementation of priority primary care initiatives within the Downtown East Toronto Ontario Health Team (DET OHT). The role includes direct people management responsibilities through oversight of primary care attachment and navigation programs and accountability for delivering projects and initiatives that implement the strategic direction of DET OHT, Ontario Health, and the Ministry of Health.
This is a non-clinical role intended for a regulated health care professional with early people-management experience and demonstrated healthcare project delivery experience. The position provides exposure to system-level planning, governance, and cross-organizational leadership.

Responsibilities and Tasks:
Program Management & People Leadership – Central Intake
- Provide direct supervision and leadership to staff supporting the primary care attachment and navigation programs
- Establish clear roles, expectations, priorities, and performance objectives for direct reports.
- Oversee day-to-day program operations, workflows, intake processes, and escalation pathways.
- Monitor program performance indicators related to access, flow, quality, and equity.
- Lead continuous quality improvement activities to enhance program effectiveness and equity.
- Ensure compliance with funding, reporting, and accountability requirements.
Project & Initiative Implementation
- Lead and coordinate healthcare initiatives aligned with DET OHT, Ontario Health, and Ministry priorities.
- Develop and maintain implementation plans, including timelines, milestones, dependencies, and resourcing.
- Monitor progress, identify risks and issues, and escalate concerns with proposed mitigation strategies.
Coordinate implementation activities across multiple partners and stakeholders.
System Planning, Governance & Reporting
- Support Executive Team and Steering Committee decision-making through preparation of briefing materials, meeting documentation, and reports.
- Participate in OHT working groups and cross-organizational committees.
- Ensure initiatives operate within established governance, accountability, and collective decision-making frameworks.
Stakeholder & Partner Engagement
- Collaborate with primary care providers, community agencies, and system partners to support integrated delivery.
- Engage family and client advisors to ensure lived experience informs program operations.
- Coordinate with internal operations teams and other OHT initiatives to support alignment and integration.
Equity & Inclusion
- Apply health equity, anti-racism, and anti-oppression principles in people leadership and program implementation.
- Use data and community feedback to identify barriers to access and inform targeted improvements.
- Foster inclusive and respectful team and partner engagement practices.
To thrive in this role, you’ll need
education/experience
- Registration as a regulated health care professional in Ontario
- Experience managing relationships, direct supervision of staff, and financial resources .
- Demonstrated healthcare project management experience.
- Experience working in primary care, community health, population health, or integrated care environments.
- Demonstrated progressive experience leading and managing complex multi-stakeholder projects, eg, system transformation, preferably in a hospital, community health or other healthcare setting.
- Experience applying design thinking principles and quality improvement methodologies.
- Experience leading and managing mid-large scale complex projects involving multiple stakeholders
Assets:
- Master’s degree in related field e.g., health administration, information technology, business administration; Public Health, Public Policy, Health Administration is an asset.
knowledge and skills:
- Demonstrated experience leading complex initiatives involving multiple and diverse stakeholders.
- Strong interpersonal, written, and verbal communication skills, with the ability to exercise tact and diplomacy, manage confidential information, and build effective working relationships.
- Working knowledge of the Ontario health system, with experience or exposure to areas such as primary care, mental health and addictions, health equity, and community-based services.
- Proficiency with standard workplace technologies, including Microsoft Office (Teams, PowerPoint, Word, Excel, Planner) and virtual meeting platforms (e.g., Microsoft Teams, Zoom).
- Ability to work both independently and collaboratively, demonstrating initiative, sound judgment, and a team-based approach.
- Strong organizational and time-management skills, with the ability to manage multiple priorities in a fast-paced environment.
- Demonstrated commitment to health equity, anti-racism, anti-oppression, and meaningful client and family engagement.
- Ability to deliver results within tight timelines and adapt to situations where outcomes and pathways may not be fully defined.
- Experience supporting or leading partnership-based projects or initiatives across organizations.

working conditions:
This position requires working in a fast-paced environment meeting the needs of multiple stakeholders. Time management, multi-tasking, initiative, organization, and prioritizing are essential. Work is mainly performed in an office environment. Occasional work outside of normal working hours may be required, in order to meet with clients, vendors and partners.
