Summary Description 

The Project Manager, Performance and Evaluation leads complex, cross-sector initiatives across the Downtown East Toronto Ontario Health Team (DET OHT). The role provides disciplined end to end project management and evaluation expertise so that clinical, digital, operational, population health and system transformation projects are planned, delivered, measured and sustained effectively. 

The position establishes clear scope, governance, workplans, schedules, resource requirements, risks, dependencies, decision processes and reporting. It works with partners to translate strategic priorities and funding commitments into practical deliverables while maintaining accountability for timelines, quality, outcomes and stakeholder engagement. 

The role also designs fit for purpose evaluation and performance measurement approaches, uses quantitative and qualitative evidence to assess implementation and impact and turns findings into decisions, improvements and recommendations. Success requires strong project leadership, sound judgment, adaptability and the 

ability to work confidently across projects with different subject matter and technical requirements. 

 

Job Functions or Duties 

 

Project and Program Management 

  • Lead projects from initiation through closeout, including clinical, digital, operational, population health and system transformation initiatives. 
  • Clarify project objectives, scope, deliverables, success measures, assumptions, constraints, roles and decision making authority. 
  • Develop and maintain project charters, integrated workplans, schedules, milestones, resource plans, budgets where applicable, risk and issue logs, dependency maps, communication plans and status reports 
  • Establish fit for purpose project governance and support sponsors, steering committees, working groups, and project teams with timely information, recommendations, decisions and escalation pathways. 
  • Coordinate work across partner organizations and disciplines, monitor progress against commitments, resolve barriers, manage scope changes, and recommend corrective action when delivery is at risk. 
  • Manage multiple concurrent priorities while maintaining clear documentation, accountability, quality and follow-through. 
  • Ensure projects align with the DET OHT Strategic Plan, Ontario Health priorities, Ministry requirements, funding agreements, privacy obligations and health equity commitments. 

Evaluation, Performance Measurement and Learning 

  • Develop practical evaluation plans, logic models, theories of change, measurement frameworks, indicators, baselines, targets, data collection methods and reporting schedules appropriate to each initiative. 
  • Use mixed methods, including quantitative data, qualitative evidence, implementation feedback, and community and provider perspectives, to assess process, outcomes, equity, experience, sustainability and impact. 
  • Build evaluation and measurement activities into the project lifecycle so that evidence informs design, implementation, course correction and closeout. 
  • Analyze and synthesize findings, identify trends, gaps, risks, and opportunities, and translate results into clear recommendations for project teams, partners and senior leaders. 
  • Support accountability reporting, funding submissions, annual planning, business cases, performance reviews and organizational priority setting. 

 

Implementation, Change and Continuous Improvement 

  • Develop implementation and change approaches that reflect the people, workflow, technology, policy, and operational impacts of each project 
  • Assess readiness, stakeholder impacts, adoption risks, training and communication needs, and supports required for implementation and sustainability. 
  • Apply project management, change management, quality improvement, implementation science and design thinking methods as appropriate to the initiative. 
  • Facilitate testing, feedback, learning, and iterative improvement; document lessons learned and support transition to operational ownership 

 

Stakeholder Engagement and Partnership Development 

  • Build and maintain trusted relationships across hospitals, primary care, community agencies, social services, Ontario Health, vendors and other system partners. 
  • Plan and facilitate collaborative sessions that clarify needs, surface risks and interdependencies, support decisions and translate diverse perspectives into aligned action. 
  • Engage providers, clients, families, caregivers, and communities in ways that are accessible, equitable and appropriate to the project. 
  • Communicate complex project, evaluation, and performance information clearly to technical and non-technical audiences through concise reports, presentations, dashboards and briefing materials. 
  • Promote shared accountability, transparent communication, and collective ownership for deliverables and outcomes.

  • Organizational Planning:  ability to lead streamlined processes to make them more efficient, and work with others to develop and adopt new procedures and processes.  Participate and/or lead in management team meetings, special committees and working groups, as required. 
  • Community Engagement: ability to foster and build strong relationships with community and funding partners, as required. Explore integrated and strategic partnerships that enhance services to the OHTs attributed population. Initiate evaluations, needs assessments, and surveys that assess emerging population needs/trends.  
  • Commitment to cross-agency integrated program planning:  ability to work co-operatively and collaboratively within the management team, working groups, or in regional/provincial work groups/ committees. This includes contributing fully and actively to team activities, projects, and program initiatives to achieve group and OHT goals.  Resolve conflicts by facilitating the expression of diverse points of view to enhance teamwork and attain a beneficial resolution. Take steps to help others resolve the conflict and maintain trusting relationships. Promote co-operation between program teams.

Qualifications:  

Skill & Abilities  

  • Undergraduate degree in a health-related discipline, health policy, business or finance 
  • Master’s degree in health administration preferred 
  • Previous experience working in a fully or partially government-funded organization required 
  • Minimum 5 years of experience in an operational, managerial role that included the management of finances and budget development 
  • Previous experience applying for and securing funding for strategic initiatives 
  • Previous experience working with multiple stakeholders across the health and social services sectors 
  • Previous experience working with both the Ministry of Health and Ontario Health required 
  • Experience working in a leadership capacity across multiple organizations required. 

 

Knowledge & Experience 

  • Experience with KPIs, measurement frameworks, dashboards, scorecards, and evaluation tools. 
  • Strong understanding of healthcare performance management and system transformation. 
  • Advanced skills in: 
  • Microsoft Excel 
  • Power BI/Tableau or similar visualization tools 
  • Microsoft Office Suite 
  • data reporting platforms 
  • Experience with SQL, statistical tools, or health datasets considered an asset. 
  • Excellent executive-level writing skills, including reports, briefing notes, presentations, and funding submissions 
  • Ability to communicate complex data concepts to non-technical audiences
