about the role:

The Registered Nurse position is part of the Integrated Recovery and COVID Care Clinics in the Downtown East OHT. The Recovery Clinics increase access to preventative care, primary care, mental health, and community support services. There will also be a focus on improving access to COVID testing, vaccines and anti-viral. The Integrated Recovery and COVID Care Clinics are in partnership with Regent Park Health Centre

The Registered Nurse assists in the provision of direct patient care and community services in order to improve the health of residents of Downtown East Toronto. The position will work out of two sites in St. James Town and Regent Park, and also from other sites as needed. This role will report to the Project Managers of the Integrated Recovery and COVID care clinics at both sites. The successful candidate must be available to work days, evenings and weekends, with the flexibility to cover additional shifts as required. They might be required to work out of different locations per identified needs. Please note the schedule and hours of work may change due to operational requirements.

day to day responsibilities:

  • Provide comprehensive nursing care for clients at the Integrated Recovery and COVID Care Clinics
  • Administer COVID testing and COVID vaccinations
  • Conduct health assessments, focused health history taking and triage for episodic or urgent health concerns
  • Implement therapeutic interventions for preventative health, health maintenance and health restoration
  • Provide health education and health promotion that are responsive to client needs
  • Collaborate with clients and families as well as the multidisciplinary team to develop client care plans
  • Lead primary care appointments to provide care including chronic disease management, smoking cessation, immunizations, well-baby/well-child visits, PAP tests, venipuncture, electrocardiogram (ECGs), sexual health counselling and testing, and TB testing
  • Facilitate client referrals to appropriate internal and external health and social services, including diagnostic services, support programs, and community-based health agencies
  • Work in collaboration with an interdisciplinary team of physicians, NPs, community ambassadors, intake workers and case managers.
  • Participate in quality improvement initiatives of the Integrate Recovery and COVID Care Clinics
  • Attend all relevant planning meetings and support with clinical flow

To thrive in this role, you’ll need

education/experience

  • Current Registration with the College of Nurses of Ontario (CNO) in the Registered Nurse – General Class
  • Baccalaureate degree in nursing (Bachelor of Science in Nursing/BScN) or diploma entry registered nurse
  • Current Basic Life Support (BLS) certification
  • 3 to 5 years demonstrated nursing experience in a community health, primary care, or similar setting
  • Experience working with at least one of Sherbourne Health’s priority populations a strong asset

skills/abilities:

  • Understand the social determinants of health and their impact on newcomer and migrant communities, homeless or underhoused individuals and members of 2SLGBTQ communities
  • Experience/interest in working with communities facing barriers to accessing health care due to poverty, social isolation, homelessness, homophobia/transphobia, racism, classism, mental health and/or substance use challenges
  • Nursing care knowledge related to primary care, community health, and chronic disease management
  • Demonstrated ability to work effectively within a multidisciplinary team
  • Solid interpersonal and crisis intervention skills
  • Effective organizational, critical-thinking, problem-solving, decision-making and communication skills.
  • Experience and comfort working with an electronic medical record (EMR)
  • Comfortable using Remote Interpretation Ontario (RIO) phone interpretation line during client assessments
  • Proven ability to problem solve effectively and independently
  • Working knowledge of Microsoft Office suite, including Word, PowerPoint, and Microsoft Teams is an asset
  • Experience with health program planning, implementation, and evaluation is an asset
  • Working knowledge of acts, legislation, and professional standards that govern nursing practice including the Personal Health Information Protection Act (PHIPA),
  • Regulated Health Professions Act (RHPA), and College of Nurses of Ontario (CNO) Standards and Guidelines
  • Ability to maintain professional practice regarding commitments to colleagues and clients, pertinent legislation, evidence-based standards of care and professional standards of practice
  • Ability to speak a second language is an asset
