Sherbourne Health seeking feedback for our next strategic plan
Since our inception in 2002, Sherbourne Health’s staff, board of directors, volunteers and community partners have worked tirelessly to develop programs and services to meet the needs of our communities. At this time, Sherbourne Health is carrying out a planning process that will help us to make decisions about our priorities for the coming three to five years so that we can best serve our communities. As part of our planning process, we would like your feedback. We want to know about your experience with Sherbourne Health and what is important to you. We would like to know what kinds of services and programs you would like to see us provide.
The information that you provide will be combined with other information that is collected during the planning process and reported without any names or identifying information. The survey will take about 10 minutes to complete, and will be open until February 25 2022.
You do not have to provide your name, but if you wish to leave your email address or phone number, you will be entered into a draw, with a chance to win one of six $50 gift cards. Please note that we do not attach the names to the surveys, and we will work to ensure your anonymity.
SN Management has been contracted to Sherbourne Health with the strategic planning process. Sonja Nerad from SN Management will be administering this survey. Your survey answers will go directly to SN Management and will not be seen by anyone at Sherbourne Health.
